The Managed Incidents System is designed to provide sites with the ability to track the occurrence and resolution of reliability incidents. This includes real incidents such as process downtime or safety incidents as well as "pseudo" incidents such as output from an ISO Quality or Environmental Audit as well as project management steps.
The Managed Incident System, based on the ISO 9000 approach to quality management, consists of 4 phases:
Phase 1: Incident Creation
Phase 2: Incident Prioritization
Phase 3: Root Cause Determination
Phase 4: Corrective Action Implementation/Verification
