RCA Step 9: Close Out

In many facilities, problems are often not completely resolved and seem to continue on year after year. Traditional troubleshooting often does little to resolve the repeat nature of this issue. Therefore with Root Cause Analysis it is usually important to communicate to the organization that the troubleshooting has been completed. This commnunication will cover :

  • the symptoms
  • the root cause
  • the solution
  • the resulting performance
  • anything else learned during the troubleshooting process

The last point is important. Often during the troubleshooting information will become available that indicates improvement opportunities not necessarily associated with the actual problem being solved. On one hand it is important to capture this information, it should not interfere with the troubleshooting process. The close-out is often a convenient place to document and share these improvement opportunities although there are other mechanisms available if desired.
